Job summary

Impact Health PC in Toms River, NJ is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric care in Skilled Nursing Facilities. You will assess, diagnose, and manage psychiatric conditions, develop individualized treatment plans, and collaborate with physicians, staff, and interdisciplinary teams to ensure high-quality patient care.

A NJ PMHNP license and a minimum of 2 years of clinical experience with adults are preferred, with experience in geriatrics or

• Active PMHNP license/certification in NJ.
• Minimum 2 years of clinical experience providing mental health care to adults.
• Experience in Skilled Nursing, Long-Term Care, geriatrics preferred.
• Strong psychiatric assessment, diagnostic, and medication management skills.

Responsibilities

• Provide comprehensive psychiatric evaluations and ongoing medication management.
• Assess, diagnose, and treat psychiatric and behavioral health conditions.
• Develop evidence-based, individualized treatment plans.
• Collaborate with physicians, nursing staff, and interdisciplinary care teams.
• Provide patient and family education regarding diagnoses, medications, and treatment plans.
• Complete timely clinical documentation and maintain compliance with applicable regulations.

Qualifications
