Job summary

Impact Health PC is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric care to residents within Skilled Nursing Facilities in West Orange, NJ.

You will assess, diagnose, and manage psychiatric and behavioral health conditions, develop individualized treatment plans, and collaborate with physicians, facility staff, and interdisciplinary care teams to ensure high-quality patient care.

Qualifications

• Active PMHNP license/certification in NJ.
• Minimum 2 years of clinical mental health experience.
• Experience in Skilled Nursing, Long-Term Care, geriatrics, hospital, residential care, or outpatient psychiatry preferred.
• Strong psychiatric assessment, diagnostic, and medication management skills.

Responsibilities

• Provide comprehensive psychiatric evaluations and ongoing medication management.
• Assess, diagnose, and treat psychiatric and behavioral health conditions.
• Develop evidence-based, individualized treatment plans.
• Collaborate with physicians, nursing staff, and interdisciplinary care teams.

• Proficiency with electronic health records (EHRs) and clinical documentation.
• Strong communication, organizational, and collaborative skills.
• Ability to practice independently within applicable state regulations while working closely with an interdisciplinary care team.
