Job summary

Impact Health PC in Toms River, NJ is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to deliver compassionate psychiatric care to adults in Skilled Nursing Facilities. You will assess, diagnose, and manage psychiatric and behavioral health conditions, and collaborate with physicians, facility staff, and interdisciplinary teams.

The role offers a flexible Monday–Friday schedule with no nights, weekends, or on-call, autonomy to practice within state regulations, and strong clinical

Qualifications

• Active PMHNP license/certification in NJ.
• Minimum 2 years clinical experience providing mental health care to adults.
• Experience in Skilled Nursing, Long-Term Care, Geriatrics, Hospital, Residential Care, or Outpatient Psychiatry preferred.
• Proficiency with electronic health records (EHRs) and clinical documentation.
• Strong communication, organizational, and collaborative skills.

Responsibilities

• Provide comprehensive psychiatric evaluations and ongoing medication management.
• Assess, diagnose, and treat psychiatric and behavioral health conditions.
• Develop evidence-based, individualized treatment plans.
• Collaborate with physicians, nursing staff, and interdisciplinary care teams.
• Provide patient and family education regarding diagnoses, medications, and treatment plans.
