Impact Health PC in Bridgewater, NJ is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric care within Skilled Nursing Facilities in the Bridgewater area. • Active PMHNP license/certification in NJ.
• Minimum 2 years of clinical mental health care experience for adults.
• Experience in Skilled Nursing or Long-Term Care preferred.
• Strong psychiatric assessment, diagnostic, and medication management skills.
• Proficiency with electronic health records (EHRs) and clinical documentation.
• Strong communication, organizational, and collaborative skills.
• Ability to practice independently within applicable state regulations while working closely with an interdisciplinary care team.
