Impact Health PC in Long Branch, NJ is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to deliver psychiatric care in Skilled Nursing Facilities. The role includes assessments, diagnoses, and ongoing medication management for adult patients, with collaboration across interdisciplinary teams.

Applicants must hold an active PMHNP license in NJ and have at least 2 years of clinical mental health experience, preferably with SNF/LTC settings.

Qualifications

• Active PMHNP license/certification in NJ.
• Minimum 2 years of clinical experience providing mental health care to adult populations.
• Experience in Skilled Nursing, Long-Term Care, Geriatrics, Hospital, Residential Care, or Outpatient Psychiatry preferred.
• Strong psychiatric assessment, diagnostic, and medication management skills.
• Proficiency with electronic health records (EHRs) and clinical documentation.
• Strong communication, organizational, and collaborative skills.
• Ability to practice independently within applicable state regulations while working closely with an interdisciplinary care team.

Responsibilities

• Provide comprehensive psychiatric evaluations and ongoing medication management.
• Assess, diagnose, and treat psychiatric and behavioral health conditions.
• Develop evidence-based, individualized treatment plans.
