Job summary
Impact Health PC in Bridgewater, NJ is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric care to residents within Skilled Nursing Facilities in the Bridgewater, NJ area.
You will assess, diagnose, and manage psychiatric and behavioral health conditions, develop individualized treatment plans, and collaborate with physicians, facility staff, and interdisciplinary care teams to ensure high-quality patient care.
Qualifications
- Active PMHNP license/certification in NJ.
- Minimum 2 years of clinical experience providing mental health care to adult populations.
- Experience in Skilled Nursing, Long-Term Care, Geriatrics, Hospital, Residential Care, or Outpatient Psychiatry preferred.
Responsibilities
- Provide comprehensive psychiatric evaluations and ongoing medication management.
- Assess, diagnose, and treat psychiatric and behavioral health conditions.
- Develop evidence-based, individualized treatment plans.
- Collaborate with physicians, nursing staff, and interdisciplinary care teams.
- Provide patient and family education regarding diagnoses, medications, and treatment plans.
- Complete timely clinical documentation and maintain compliance with applicable regulations.
