Qualifications

Responsibilities

• Assess patient condition and coordinate nursing care with the treatment team.
• Develop and implement individualized care plans including discharge planning.
• Administer direct care and medications as prescribed.
• Practice infection control and maintain a safe environment.
• Communicate patient status changes to Charge RN/Clinician.
• Develop and maintain written plans of care and set patient/family goals.
• Document patient/family teaching and responses in medical records.
• Collaborate with staff and departments; participate in quality improvement activities.
