Job summary

Columbus House, Inc. is seeking a Full Time (32 hours/week) PSH Case Manager for Shoreline Grove in Branford, CT. The role focuses on housing placement, connection to medical and social services, and tenancy support for adults experiencing homelessness.

Key responsibilities include coordinating housing resources, maintaining documentation, and collaborating with landlords and service providers. A valid driver’s license is required and the position offers comprehensive benefits.

Qualifications

• Excellent organizational and time management skills with a deadline-oriented mindset.
• Strong written and oral communication skills employing the English language.
• Proficient in Microsoft Office and other common desktop and online applications.
• Knowledge of and ability to effectively navigate substance abuse, mental health, and social services networks.
• Familiar with criminal justice barriers and able to engage, motivate, and support the individuals serviced.
• Able to work with multiple populations including youth (18-24 years old), adults, and families.
• Understanding housing first principles, housing law, tenant rights/responsibilities
• Valid drivers’ license with a good driving history

Responsibilities

• Assist clients with finding, securing, and maintaining housing.
• Provide linkage to resources for clients at risk of eviction and for social, mental health, and entitlements services.
• Communicate efficiently with individuals served.
• Advocate for clients and support self-advocacy.
• Maintain boundaries and confidentiality.
• May provide transportation to appointments as needed.
• Coordinate with other agencies and providers.
• Develop relationships with landlords and housing resources.
• Understand varied backgrounds and maintain non-judgmental service delivery.
• Attend meetings and facilitate tenant meetings.
• Cover vacancies as needed.
• Complete high-quality documentation and assessments.
• Conduct health and safety checks at least every 6 months.
