Job summary

The Centers is hiring a Case Manager for Permanent Supportive Housing in the Cleveland area. The role supports the PSH Care Team across multiple locations, focusing on strength-based services and holistic client care to promote housing stability.

You will develop service plans, apply Housing First and Harm Reduction principles, and partner with a multidisciplinary team to link clients with essential resources and supports.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree in Social Work, Psychology or related field preferred.
• Valid Ohio driver's license and insured vehicle for client transport.
• Nonviolent Crisis Intervention certification within 60 days of hire.
• Active Community Health Worker certification preferred.
• At least three years of case management or care coordination experience.

Responsibilities

• Provide strength-based case management to individuals with chronic homelessness and behavioral health needs.
• Develop individualized service plans and housing success plans with measurable outcomes.
• Coordinate with a multidisciplinary team to connect clients to primary care, behavioral health, and housing resources.
• Engage clients, conduct assessments, and support independent living skills and budgeting.
• Document case management activity in EMR and ensure linkage to client goals.
• Support onsite food pantry operations and maintain food safety certification as required.
