Job summary

Columbus House in New Haven, CT is seeking a CCR Case Manager to provide case management for adults experiencing homelessness who will live in long-term supportive housing. The role focuses on housing placement, linkage to medical, mental health, and employment resources, and community collaboration.

This full-time (32 hours/week) position offers a hourly wage of $25.50–$27.50, benefits including medical, dental, vision, a 401(k) plan with employer matching, paid time off and holidays.

Qualifications

• Excellent organizational and time management skills with a deadline-oriented mindset.
• Strong written and oral communication skills in English.
• Proficient in Microsoft Office and common desktop/online apps.
• Knowledge of and ability to navigate substance abuse, mental health, and social services networks.
• Familiar with housing first principles and tenant rights/responsibilities.

Responsibilities

• Assist clients with finding, securing, and maintaining housing.
• Provide linkage to resources for clients at risk of eviction and connect to social services.
• Communicate efficiently with individuals served and advocate for clients.
• Coordinate with landlords and other agencies to facilitate housing moves.
• Complete required documentation and reports for CARF DHMAS funded programs.
• Attend meetings and provide supervision coverage as needed.
