Job summary

The New London Homeless Hospitality Center is seeking a full-time Permanent Supportive Housing (PSH) Case Manager to join our Intensive Services Department in New London, CT. The role focuses on helping formerly homeless individuals achieve long-term housing stability through individualized service plans and coordinated community resources.

Location includes New London and Groton; schedule is 40 hours per week, Monday through Friday.

Qualifications

• Interpersonal communication skills required.
• Strong conflict resolution and problem-solving abilities.
• Knowledge of CT housing laws and benefits administration.
• Experience with documentation and file management.

Responsibilities

• Travel to client apartments and conduct regular home visits.
• Complete initial orientation and assessments for new participants.
• Create client-centered service plans and implement them.
• Record all services and referrals per NLHHC policy.
• Assist clients with government entitlements, disability benefits, food stamps, and rental assistance papers.
• Coach clients on lease compliance and rent payments.
• Support property managers with paperwork and tenant issues as needed.
• Maintain contact with landlords to protect housing stability.
• Maintain accurate case files and progress reports.
• Participate in case conferences and follow up on issues.
• Provide crisis intervention when necessary.
