Job summary

Qualifications

• Excellent communication and presentation abilities.
• Proven ability to build long-term provider relationships.
• Experience with STARs measures and health-plan programs.
• Willingness to travel up to 75% locally.
• Knowledge of electronic medical records is a plus.

Responsibilities

• Lead provider performance management using designated metrics (e.g., 4 STAR gap closure).
• Educate and report to providers; identify performance improvement opportunities.
• Travel across territory to meet with providers and discuss tools and programs.
• Execute provider incentive programs for the health plan.
