Job summary

Mercy Home Health in Ohio is seeking a Registered Nurse (RN) for Progressive Care (Step down). The role emphasizes patient assessment, developing care plans, and serving as the liaison between Med-Surg and ICU to prioritize needs.

You will administer medications safely, collaborate with the care team, and participate in quality improvement initiatives. BSN is preferred but not required; an active RN license and BLS/ACLS as noted are required or preferred.

Qualifications

• 1 year of RN experience is required.
• 6 months RN experience in an acute care hospital setting is preferred.
• Current state licensure as a Registered Nurse (RN) is required.

Responsibilities

• Collaborate with the interdisciplinary care team.
• Conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment, and evaluates treatment effectiveness.
• Serves as point of contact for patients' care coordination throughout hospital departments.
• Acts as a patient safety advocate by participating in ongoing quality improvement in the department.
• Administer medications in a safe manner compliant with policies.

Bachelor of Science in Nursing (BSN) — preferred
