Job summary

Abound Health PA, LLC in Bellefonte, Pennsylvania seeks a Program Specialist to oversee and support services for individuals under Pennsylvania Waiver or Base Funded Services. You will supervise direct care staff, participate in ISP implementation, and ensure documentation quality.

The role emphasizes coordinating client goals, assessments, progress notes, and safe, compliant service delivery across home and community settings, including travel within the region.

Qualifications

• Experience working with individuals with intellectual and/or developmental disabilities preferred.
• Knowledge of ISP implementation, task analysis, goal writing, and documentation practices.
• Strong organizational, communication, and interpersonal skills.
• Ability to manage multiple priorities and work independently and collaboratively.

Responsibilities

• Oversee client services, documentation, and records to ensure compliance with applicable ODP requirements.
• Supervise and support direct care staff, including performance evaluations, development, training, and scheduling.
• Participate in ISP meetings and support implementation of client goals, task analyses, and interventions.
• Complete assessments, progress reports, service documentation, and incident reports as required.
• Monitor service utilization, billing, staffing coverage, and service authorizations.
• Ensure client-specific training is completed and provide guidance to direct care staff.
• Support clients and families with accessing appropriate community resources and services.
