Job summary

Kahak Health Care Services seeks a dedicated supervisor to oversee home-based supports for individuals with developmental disabilities in Indiana. Responsibilities

Job description

Kahak Health Care Services is a service provider for individuals with intellectual and developmental disabilities. We provide in-home support services, community coaching, and respite care. Responsibilities

• Overseeing and enhance program operations and digital systems to support strategic goals of the organization
• Coordinating with and supervising Direct Support Professionals that provide support for your caseload of individuals. This includes monthly supervisory calls, visits, and review of documentation and clock-in and out times, as well as performance reviews to ensure safety, ethical, and standards of care are in compliance with State regulations and company policy and procedure.
• Collaborating with Support Coordinators/Case Managers/CSWs of individuals on your caseload to ensure appropriate and effective provision of supports. This includes attending Individual Support Plan (ISP) meetings (in person or virtually), reporting incidents, and strategizing for the best care solutions of each individual.
• Communicating to DSPs accurate policies, updates, and providing ISP and annual trainings.
• Provide monthly communication with parents/guardians/families of the individuals on your caseload to encourage effective communication and collaboration.
• Creating and maintaining individuals’ binders for those on your caseload, keeping updated information including Part V reports, quarterly reports, current medical information, annual physical exams and dental exams, and all consent forms.
• Attend all relevant meetings, training sessions, and professional development seminars and classes.
• Ensure all required reports are completed and submitted in a timely manner in compliance with state and company regulations and policies.
• Foster and promote a person-centered approach to provision of support services in which the choices, dignity, and independence of each individual on your caseload is considered and respected.

Educational Requirements

A high school diploma or equivalency is required with a minimum of 1 year of experience in human services (social work, psychology), intellectual disabilities, developmental disabilities, the medical field, healthcare administration, or other related field.
Supervisory or management experience preferred.

An Indiana Police Report, a tuberculosis screening with a negative result, and an Indiana driver's license is required.
