Job summary

Kahak Health Care Services seeks a dedicated supervisor to oversee home-based supports for individuals with developmental disabilities in Indiana. Qualifications

• High school diploma or equivalent with 1 year in human services.
• Strong organizational and leadership skills.
• Excellent communication with families and professionals.

Responsibilities

• Overseeing and enhance program operations and digital systems.
• Coordinate with Direct Support Professionals for caseloads, monthly supervision, visits, and clock-in/out reviews.
• Collaborate with Support Coordinators/Case Managers/CSWs to ensure ISP meetings and best care solutions.
• Communicate policies and provide ISP and annual trainings to DSPs.
• Provide monthly communication with families to encourage collaboration.
• Create and maintain individuals’ binders with up-to-date medical and consent documents.
• Attend meetings, trainings, and professional development sessions.
• Ensure timely submission of required reports in compliance with regulations and policies.
• Foster a person-centered approach that respects choices, dignity, and independence.
