Helpful Hearts Health Services Inc. in Maryland is seeking a dedicated Program Coordinator to support day-to-day operations of residential and community programs. The role emphasizes hands-on field work, compliance with DDA/COMAR/CMS, and strong teamwork.

You will oversee homes, support staff, ensure person-centered service delivery, and help maintain documentation, meetings, and follow-up. Occasional evenings, weekends, and on-call duties may be required.

Formación

• Associate degree in Human Services or related field.
• At least 3 years of experience supporting individuals with developmental disabilities.
• Valid driver's license and reliable transportation.
• Ability to lift up to 50 lbs.
• Strong communication and documentation skills.

Responsabilidades

• Support individuals in homes and communities to promote safety and participation.
• Assist with PCP goals, outcomes, and strategies.
• Attend annual PCP and team meetings.
• Monitor progress and follow-up.
• Provide coaching and scheduling support to staff.
• Complete required documentation accurately and timely.
• Maintain professional communication with team and families.
• Ensure compliance with agency, DDA, COMAR, CMS.
