Job summary

Kids for the Future in Holmes, PA is seeking a Program Lead to manage individualized service plans for people with intellectual and developmental disabilities. The role requires a bachelor’s degree in a human services field and at least two years of relevant experience.

You will coordinate assessment, monitor progress, and work with an interdisciplinary team, licensing, and family networks to ensure Active Treatment and compliant documentation.

Qualifications

• Bachelor’s degree in a human services field.
• Two years experience working with people with Intellectual Disabilities.
• Must communicate effectively in English.
• Working knowledge of MS Word, Excel, Outlook, and basic knowledge of Access/databases.

Responsibilities

• Participates in intake and admission of new individuals.
• Functions as Plan Lead for development and monitoring of each individual's program plan.
• Supervises, monitors and evaluates services, equipment, and programs to meet needs and ensure Active Treatment.
• Coordinates completion of initial and subsequent assessments for each case load.
• Monitors progress of goals and documents monthly updates.
