This role involves developing individualized support plans, monitoring implementation, and ensuring regulatory compliance while maintaining confidential documentation and preparing program reports.

Qualifications

• Strong written and verbal communication skills.
• Experience coordinating assessments and developing support plans.
• Familiarity with regulatory documentation and compliance.

Responsibilities

• Develop, oversee, and update comprehensive assessments for each individual.
• Collaborate with teams to create and monitor Individual Support Plans.
• Educate staff and monitor implementation of support strategies.
• Support teams during behavioral incidents, psychiatric hospitalizations, and skill training.
• Conduct effective team meetings and serve as point person for investigations.
• Ensure consistency and regulatory compliance of all clinical documents.
• Assist with licensing audits and required documentation.
• Maintain confidentiality and adhere to all regulatory standards.
• Complete financial reconciliations and monitor program expenditures.
• Prepare program reports and statistical data.
• Develop and maintain collaborative relationships with supervisors and peers.
• Seek and participate in professional development opportunities.
