Job summary

Kids for the Future in Springfield, PA is seeking a full-time exempt professional to lead program planning and service delivery for individuals in our care. You will coordinate intake, develop and monitor individualized plans, and work with interdisciplinary teams to ensure active treatment and quality outcomes.

The role requires a 4-year degree, strong collaboration with residential, nursing, day program and family stakeholders, and participation in licensing inspections and training

Qualifications

• Requires a 4-year degree and relevant professional competencies.

Responsibilities

• Participates in the intake and admission process of new individuals.
• Functions as the Plan Lead in the development, implementation and monitoring of each individual’s program plan.
• Supervises services, equipment, and programs to meet individuals’ needs and ensure Active Treatment.
• Coordinates initial and ongoing assessments; tracks progress against goals and documents quarterly reports.
