Job summary

HealthMark Rehabilitation is seeking PRN Speech Therapists/CCC-SLPs to treat residents and short-term patients at our Harbor Lakes facility in Granbury, Texas. The role requires Texas CCC-SLP licensure and focuses on geriatric rehabilitation in a SNF setting.

Responsibilities include developing care plans, delivering skilled therapy, documenting progress, and ensuring compliant billing and infection control. Strong communication and organizational skills are required.

Qualifications

• Must hold Texas license/certification for CCC-SLP.
• Strong oral and written communication skills; organizational and analytical skills required.
• Ability to interact effectively with staff and customers; time management and organization.
• Experience in geriatric rehabilitation preferred.

Responsibilities

• Provides therapy services in accordance with physician’s orders and accepted professional practices.
• Develop an overall plan of care including treatment and medical diagnosis warranting skilled intervention; planned duration and frequency; measurable goals and discharge plan.
• Therapists of all disciplines perform skilled treatments according to established plan of care.
• Documents observations, patient responses, progress and changes within defined timeframes.
• Completes all documentation timely and in accordance with state and federal regulations and policies/guidelines.
• Completes all billing on the date of provided skilled services in accordance with regulations and guidelines.
• Assist with maintaining rehab gym equipment and treatment area with infection prevention practices.
