Job summary

HealthMark Rehabilitation is seeking PRN Physical Therapists to treat residents and short-term patients at our Harbor Lakes facility in Granbury, Texas. The role requires a Texas PT license, strong communication, organizational and analytical skills, and the ability to work in a multidisciplinary team.

Geriatric rehab experience preferred; background check will be completed. Location: 1300 E 2nd St, Granbury, TX 76048.

Qualifications

• Must hold Texas license/certification for PT/OT/ PTA/CCC-SLP.
• Experience in geriatric rehabilitation preferred.
• Successful completion of hiring process / background check.
• Strong communication and documentation skills.

Responsibilities

• Provides therapy services in accordance with physician's orders and accepted professional practices.
• Develops a plan of care with treatment goals, duration, and frequency to support discharge planning.
• Performs skilled treatments according to established plan of care.
• Documents observations and patient responses within required timeframes.
• Completes billing for provided skilled services in compliance with regulations.
• Maintains rehab gym equipment and treatment area with infection prevention methods.
