Job summary

Pleasant Valley Healthcare in Garland, TX, is seeking a full-time Speech-Language Pathologist (CCC-SLP) to join our in-house rehab program. The role involves screening, evaluation, and delivering rehabilitative therapies to geriatric patients in a post-acute setting.

Qualifications

SLP license required.
CCC credentials considered.
SNF experience preferred.

Responsibilities

Conduct screening and evaluation of patients with physical functioning disorders and/or wound care; develop care plans.
Deliver rehabilitative, skilled, and medically necessary treatments per standards.
Assist with activities of daily living and patient transfers as needed.
Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
Provide counseling to patients, families, caregivers, and other providers.
Generate discharge summaries compliant with regulatory and clinical requirements.
