Job summary

Nans Pointe Rehabilitation and Nursing in Suffolk, VA is seeking a PRN Speech-Language Pathologist to join our rehabilitation team. You will provide skilled speech, language, cognitive, and swallowing services to residents to improve communication and quality of life.

You will evaluate residents, develop individualized treatment plans, monitor progress, educate families, and collaborate with the interdisciplinary team while maintaining HIPAA compliance.

Qualifications

• Strong clinical, communication, documentation, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Long-term care or geriatric experience preferred but not required.

Responsibilities

• Evaluate residents referred for speech-language pathology services.
• Assess communication, cognitive, speech, language, and swallowing needs.
• Develop and implement individualized treatment plans with measurable goals.
• Provide skilled therapy per physician orders and plans of care.
• Treat residents with speech, language, cognitive-communication, and swallowing disorders as appropriate.
• Monitor progress and adjust treatment approaches based on outcomes.
• Complete evaluations, daily treatment notes, progress notes, re-certifications, and discharge documentation.
• Educate residents, families, caregivers, and staff on therapy goals and recommendations.
• Collaborate with nursing and the rehabilitation team to promote coordinated care.
• Make recommendations on diet, swallowing safety, and communication strategies.
• Maintain confidentiality and comply with HIPAA and applicable regulatory requirements.
• Perform additional duties as assigned by the Director of Rehabilitation.
