Job summary

Nans Pointe Rehabilitation and Nursing in Suffolk, VA is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis. You will deliver skilled speech, language, cognitive, and swallowing services to residents, with emphasis on independence and quality of life.

As part of an interdisciplinary team, you will evaluate and treat residents, develop measurable goals, educate families, and collaborate on patient-centered plans.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Current and valid SLP license/registration in the state.
• Long-term care experience preferred.
• Strong communication and organizational skills.

Responsibilities

• Evaluate residents referred for SLP services.
• Assess communication, cognitive, speech and swallowing needs.
• Develop and implement individualized treatment plans with measurable goals.
• Provide therapy per physician orders and plans of care.
• Treat speech, language, cognitive-communication, and swallowing disorders.
• Monitor progress and adjust treatment approaches.
• Complete evaluations, daily notes, progress notes and discharge docs.
• Educate residents, families, and staff on goals and strategies.
• Collaborate with nursing and interdisciplinary team.
• Recommend diet, swallowing safety and communication strategies.
• Maintain confidentiality and HIPAA compliance.
• Perform additional duties as assigned.
