Job summary

Creative Solutions in Healthcare, Inc. in Texas seeks a compassionate Physical Therapy Assistant (PTA) to join our in-house team on a PRN basis.

You will provide person-centered care to help residents improve mobility, strength, and quality of life under the supervision of a Physical Therapist. Candidates must hold an active Texas PTA license, be in good standing with regulatory boards, and have SNF/long-term care experience.

Qualifications

• Active and valid Texas PTA license.
• In good standing with regulatory agencies.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain and improve function.
• Provide skilled PT services under the supervision of a Physical Therapist.
• Respond to service requests and coordinate with the PT.
• Implement and modify plans of care under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient response and adjust approaches with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Ensure thorough documentation compliant with policies and regulations.
• Attend required meetings designated by the Director of Rehab.
