Job summary

Evolve Therapy Services in Milford, NE is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility on a PRN basis. Under the supervision of the Physical Therapist, you will provide services, monitor progress, and help residents regain mobility and functional independence.

This role emphasizes collaborative care, ongoing documentation in NetHealth EMR, and education for residents, families, and staff.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and in developing treatment plans with the Physical Therapist.
• Implement treatment plans including mobility, strength, balance, gait training, and modalities.
• Document CMS CMS expectations in NetHealth EMR.
• Participate in resident, family, and staff education.
• Maintain scheduling and ensure productivity.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license
