Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to join our in-house team at the Milford, NE facility. You will work under the supervision of a Physical Therapist to deliver patient-centered care and support treatment plans for residents.

The role focuses on screenings, mobility and gait training, therapeutic activities, and documentation in NetHealth EMR. Requires an Associate's degree and an active PTA license; licensed professionals encouraged to apply.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide PT services under supervision of a Physical Therapist.
• Screen residents to determine need for intervention.
• Assist in evaluations and treatment planning with PT supervision.
• Conduct group and individual treatments including mobility and gait training.
• Document CMS-compliant records in NetHealth EMR.
