Job summary

Evolve Therapy Services is recruiting a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Grand Island, NE. This PRN role supports residents under the supervision of a Physical Therapist and helps maintain quality therapy services.

Responsibilities include screenings, developing treatment plans, and delivering individual and group therapy focusing on mobility, strength, gait, and balance, while documenting care in NetHealth EMR.

Qualifications

• Must have an Associate's degree in physical therapy.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assume responsibility for ongoing continuing education and professional development.
• Experience with NetHealth EMR documentation is preferred.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention.
• Assist in the evaluation process under the direction of the Physical Therapist.
• Assist in identifying targets and goals within the treatment plan.
• Conduct both individual and group treatment focusing on strength, mobility, balance, gait, and ROM.
• Adapt techniques/media as indicated by resident performance in consultation with the Physical Therapist.
• Monitor residents’ response to intervention and modify as indicated.
• Coordinate schedules with the Physical Therapist and care team.
• Educate residents, families, and staff on therapy use.
• Participate in Quality Assurance and CMS documentation expectations in NetHealth EMR.
