Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility located in North Platte, NE. This Full Time, Part Time or PRN role offers a $5,000 relocation or sign-on bonus with flexible scheduling.

You will assist the Physical Therapist in planning and delivering services, conducting screenings, and supporting patient education while maintaining CMS-compliant documentation.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must hold an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and in developing target goals and treatment plans with the Physical Therapist.
• Carry out treatment plans including strength, mobility, balance, gait, ROM and modalities.
• Train residents, families and staff in use of equipment and participate in education.
• Maintain compliant and medically necessary documentation per CMS expectations in NetHealth EMR.
• Coordinate schedules with the Physical Therapist and team members.
