Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Maintain ongoing continuing education to sustain licensure and certification.

Responsibilities

• Provide physical therapy services following established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate, plan, implement, and follow up resident therapy programs.
• Screen residents regularly to determine need for intervention or treatment.
• Develop treatment plans with short- and long-term goals to meet identified outcomes.
• Communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
