Job summary

Creative Solutions in Healthcare - South is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis. You will provide person-centered care to help residents improve mobility and quality of life under the supervision of a Physical Therapist.

The role requires an active Texas PTA license, SNF or long-term care exposure, and familiarity with Net Health. We offer flexible schedules and competitive compensation, plus a collaborative team environment and

Qualifications

Active and valid Texas PTA license.
In good standing with regulatory agencies.
SNF or long-term care experience preferred.
Knowledge of PT treatment practices.
Experience with Net Health EHR preferred.

Responsibilities

Administer medically prescribed physical therapy treatments under PT supervision.
Provide PT services and interventions per physician orders.
Relay information and referrals to the Physical Therapist on request.
Implement and modify plans of care under PT supervision.
Measure and record motor function, strength, and muscle performance.
Monitor patient response and adjust approaches with the team.
Provide patient, family, and caregiver education.
Ensure documentation complies with policies and regulations.
Attend required Rehab Director meetings.
