Job summary

Graham Oaks Care in Graham, TX is seeking a compassionate Physical Therapy Assistant (PTA) on a PRN basis to help residents improve mobility and quality of life. You will perform medically prescribed treatments under a Physical Therapist's supervision and contribute to person-centered care.

This role requires an active Texas PTA license, experience in SNF or long-term care preferred, and familiarity with Net Health Electronic Documentation System.

Qualifications

• Active and valid Texas PTA license.
• Experience in SNF or long-term care preferred.
• Knowledge of physical therapy treatment practices.

Responsibilities

• Administer medically prescribed physical therapy treatments under the supervision of a Physical Therapist.
• Provide skilled physical therapy services and interventions per physician orders.
• Respond to requests for service by relaying information to the Physical Therapist.
• Implement and modify patients’ plans of care under PT supervision.
• Measure and record patients’ motor function, strength, and muscle performance.
• Monitor patient response to treatment and adjust as needed with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Ensure timely and thorough documentation per policies and regulations.
• Attend required Rehab Director meetings.
