Job summary

Huebner-Creek-Health- is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis. You will administer PT treatments under a PT, document care accurately, and support residents in improving mobility and quality of life.

You should hold an active Texas PTA license, have knowledge of PT practices, and be comfortable with Net Health documentation. Prior SNF or long-term care experience is preferred, with a focus on collaboration and patient-centered

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• In good standing with all regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide skilled physical therapy services and interventions under the supervision of a Physical Therapist.
• Respond to requests for service by relaying information and referrals to the Physical Therapist.
• Implement and modify patients' plans of care under PT supervision.
• Measure and record patients' motor function, strength, and muscle performance.
• Monitor patient response to treatment and modify approaches with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Ensure documentation is thorough and timely per company policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
