Job summary

Creative Solutions In HealthCare is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis in Texas. You will support residents by delivering prescribed PT, under PT supervision, promoting mobility and quality of life.

The role emphasizes flexibility with Sundays, Mondays, Fridays preferences, strong communication, and collaborative care with the rehab team. Prior SNF exposure is preferred, and Net Health proficiency is a plus.

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide skilled physical therapy services and interventions under the supervision of a Physical Therapist.
• Respond to requests for service by relaying information and referrals to the Physical Therapist.
• Implement and modify patients’ physical therapy plans of care under PT supervision.
• Measure and record patients’ motor function, strength, and muscle performance.
• Monitor patient response to treatment and adjust approaches with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Ensure documentation is thorough and timely in line with policies and regulations.
• Attend required meetings designated by the Director of Rehab.
