Job summary

Graham Oaks Care Center in Graham, TX is seeking a compassionate Physical Therapy Assistant (PTA) on a PRN basis to join our in-house rehab team. You will administer PT treatments, assist with mobility goals, and support interdisciplinary care under a PT supervisor.

Required is an active Texas PTA license and knowledge of PT practices; SNF experience is preferred, with familiarity in Net Health documentation. This role offers flexible PRN scheduling and a collaborative work environment.

Qualifications

• Current Texas PTA license.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.
• Familiarity with Net Health Electronic Documentation System preferred.

Responsibilities

• Administer medically prescribed physical therapy treatments under PT supervision.
• Provide skilled PT services in accordance with physician orders.
• Respond to service requests and relay information to PT.
• Implement and modify plans of care under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses and adjust treatment as needed with the team.
• Provide education to patients, family members, and caregivers.
• Ensure timely documentation per policies and regulations.
• Attend meetings as designated by the Director of Rehab.
