Job summary

CommuniCare Rehab, part of the CommuniCare Family of Companies, is seeking a PRN Physical Therapist (PT) for Bridgeport Healthcare Center in Bridgeport, WV. In-house team members provide direct therapy to residents in a growing multi-site system across IN, MD, OH, PA, VA, and WV.

The role emphasizes hands-on evaluation and treatment, interdisciplinary collaboration, and opportunities to work across centers to meet diverse resident needs.

Qualifications

• Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, Review, and Screen patients/residents’ medical charts, evaluations under physician order and develop a treatment plan.
• Implement treatment plan through direct treatment and education of the plan of care to all team members, including supervising PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to rehab team and train staff/family as appropriate.
• Opportunity to work at multiple centers to serve a wide variety of patient needs.
