Job summary

CommuniCare Rehab, part of the CommuniCare Family of Companies, has a PRN opening for a Physical Therapist at BridgePort Healthcare Center. This role involves evaluating patients, developing and implementing treatment plans, and leading interdisciplinary care with PTAs and staff.

The position supports a flexible work schedule with opportunities to work across centers, focusing on delivering high-quality, person-centered care to residents.

Qualifications

• Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
