Job summary

CommuniCare Rehab is seeking a PRN Physical Therapist for the Berkeley Springs Healthcare Center in Berkeley Springs, WV. The role supports our in-house rehab team and has the potential to work across multiple centers, delivering direct therapy and care planning to residents.

Applicants must hold a degree in Physical Therapy from an accredited program and a valid or pending state license. This PRN position offers flexible scheduling and competitive compensation with a strong focus on quality

Qualifications

• Physical Therapist degree from an accredited program.
• Valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, review, and screen patients’ resident medical charts and evaluations under physician order and develop a treatment plan.
• Implement treatment plans through direct treatment and education of the plan of care to all team members, including supervising PTAs.
• Lead interdisciplinary meetings to relay clinical findings and make rehab team recommendations and train staff/families as appropriate.
• Opportunity to work at multiple centers to serve a wide variety of patient needs.

Education

Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program

Qualifications:

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:
