Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a PRN Physical Therapist (PT) opening at Lincoln Healthcare Center, where therapists are In-House Team Members. This role offers flexibility and a direct impact on residents’ therapy outcomes.

The position requires a PT degree from an accredited program and an active or pending state license. You may work at multiple centers, delivering patient-centered care and collaborating with the rehab team.

Qualifications

• Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Responsibilities

• Evaluate, review, and screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
