Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a PRN opening for a Physical Therapist (PT) at Moundsville Healthcare Center, where our therapists are In-House Team Members.

This position involves evaluating residents, implementing treatment plans, and educating team members; supervision of PTAs may be required, with opportunities to work across multiple centers to meet diverse patient needs.

Qualifications

• Must hold a Physical Therapy degree and licensure as applicable in WV.
• Experience with patient evaluation and plan-of-care development is beneficial.

Responsibilities

• Evaluate, review, and screen residents' medical charts and evaluations; develop treatment plans.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
