American Senior Communities is seeking a PRN Physical Therapist to join Monticello Healthcare and Hickory Creek Winamac. You will deliver skilled physical therapy to residents, evaluate patients, and participate in treatment planning and discharge planning.

You will supervise Physical Therapy Assistants, contribute to case management, and collaborate with center staff while adhering to state regulations. This role offers flexible shifts and meaningful work serving senior residents across ASC

The Physical Therapist implements skilled physical therapy services for patients in all ASC Therapies & Wellness settings, in accordance with the principles and practices of physical therapy and within the policies and procedures.
