Job summary

American Senior Communities in Monticello, IN seeks a Physical Therapist to deliver skilled services across ASC Therapies & Wellness settings, aligning with PT standards and center policies.

You will evaluate patients, develop and update treatment and discharge plans, supervise PTAs, contribute to case management, and maintain positive interactions with residents and staff, while adhering to state regulations.

Qualifications

• Provide skilled physical therapy services following professional standards and center policies.
• Evaluate patients and develop initial and ongoing treatment plans.
• Develop discharge plans and coordinate care transitions.
• Supervise Physical Therapy Assistants (PTA) as assigned.
