Job summary

Responsibilities

• Evaluates patients and establishes patient care plans with modalities, exercises, and education.
• Gathers data from medical records and reports to inform care and device needs.
• Documents all evaluations, tests, progress notes, education, discharge notes, and charges.
• Re-evaluates and updates goals as patient status changes.
• Coordinates care with physicians and staff and schedules treatment and discharge planning.
• Provides patient/family education on deficits and home programs.
