Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.

Responsibilities

• Evaluates patients and establishes appropriate patient care plan.
• Gathers and interprets data from medical records and reports.
• Documents evaluations, tests, progress notes, and discharge plans.
• Re-evaluates patients as status changes and adjusts goals.
• Communicates progress to physician, staff and family; coordinates treatment and discharge planning.
• Provides patient and family education with home program instructions.
• Directs and supervises Physical Therapist Assistant and support staff.
