The role requires a PT degree, state license, and CPR/BLS, with responsibilities spanning evaluation, treatment planning, documentation, and coordination with the care team to support patient progress.

Qualifications

• Bachelor's degree in PT from an accredited program.
• State license to practice PT in the state.
• CPR and BLS certifications required per policy.
• Experience in outpatient PT settings is a plus.

Responsibilities

• Evaluates patients and establishes care plans with modalities and education.
• Gathers data from medical records and coordinates with physicians and staff.
• Documents evaluations, progress notes, and discharge summaries.
• Re-evaluates patients and adjusts goals as needed.
• Communicates progress to physicians and care team; coordinates schedules and discharge planning.
• Educates patients and families on home exercise programs and equipment use.
• Supervises PT assistants and trainees as needed.
