Responsibilities

• Evaluates patients and establishes care plans with appropriate modalities and exercises.
• Gathers data from medical records and families to inform treatment.
• Sets functional goals with patients and provides ongoing evaluation of progress.
• Documents evaluations, progress notes, and discharge planning.
• Communicates progress to physicians and care teams; coordinates schedules and treatments.
• Supervises PTA and Rehab Tech/Aide as required for licensure.
