Atrium Health is seeking a licensed Physical Therapist to join our inpatient rehabilitation team on a PRN basis at Carolinas Rehabilitation locations in the Charlotte area. This position is for interest in PRN opportunities and is not a float or travel role.

You will evaluate patients, establish care plans with modalities, gather data from medical records and staff, document evaluations and progress, re‑evaluate as needed, and communicate progress to physicians.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program is required.
• Applicable state license to practice PT is required.
• CPR Certification and BLS are required per policy guidelines.

Responsabilités

• Evaluates patients and establishes appropriate PT care plans including modalities and exercises.
• Gathers and interprets data from medical records, physician reports, patient and family; identifies need for assistive devices.
• Documents evaluations, tests, progress notes, and discharge planning; tracks outcomes and charges.
• Re-evaluates patients as status changes and modifies goals accordingly.
• Communicates evaluation results and progress to physicians and staff; coordinates scheduling and discharge planning.
• Provides patient/family education on deficits, progress, and home programs; ensures equipment use guidance.
• Directs and supervises PT Assistant, PT Technician, students and volunteers; helps orient new staff.
