You will evaluate patients, plan care, monitor outcomes, and collaborate with caregivers and physicians to ensure optimal function and safety across settings.

Qualifications

• Licensed Physical Therapist in the state of employment.
• BLS certification for Healthcare Providers.
• Continuing education requirements as mandated by state standards.
• Doctor of Physical Therapy (DPT) preferred or education aligned with graduation date.

Responsibilities

• Provide age-appropriate patient care across the life span.
• Evaluate patients and develop evidence-based care plans.
• Set functional goals and monitor outcomes with patient/caregiver input.
• Educate patients and caregivers on diagnosis, treatment, and home programs.
• Communicate professionally with patients, caregivers, and staff.
• Complete documentation including evaluations, progress notes, and billing.
• Maintain a clean, organized department and proper equipment care.
• Pursue professional development and apply evidence-based practices.
• Engage in performance improvement and coordinate coverage across locations.
• Float across departments and adapt to varying clinical environments.
