Qualifications

• Bachelor's degree in Physical Therapy is required.
• Applicable state license for Physical Therapy is required.
• BLS certification required per policy guidelines.

Responsabilités

• Evaluates patients and establishes a care plan with modalities and education.
• Gathers and interprets data from the medical record, physician reports, patient, and family.
• Establishes functional goals and provides evidence-based intervention toward meeting these goals.
• Documents evaluations, tests, progress notes, discharge notes and charges as needed.
• Coordinates patient care and discharge planning; communicates progress to physicians and staff.
• Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
